Eyebrow Transplant — permanently full brows

Thin, sparse, or missing eyebrows—whether due to overplucking, scars, alopecia, or age-related hair loss—can be permanently restored with a transplant. This is the ultimate discipline of hair transplantation.

Who Benefits from Eyebrow Transplants

Eyebrows are one of the most defining facial features. When they thin or are absent, it changes the entire dynamic of facial expression. A transplant permanently restores them.

Common Indications

  • Over-plucking consequences — years of intense plucking can permanently inactivate some follicles
  • Scars from injuries, burns, or surgeries in the brow area
  • Alopecia areata barbae — patchy hair loss in the beard or eyebrow area
  • Frontal fibrosing alopecia (FFA) — scarring alopecia, common in postmenopausal women
  • Age-related thinning — natural loss from 50–60 years of age
  • Chemo or radiation therapy aftermath
  • Trichotillomania — compulsive hair pulling (after treatment of the underlying condition)
  • Congenitally missing eyebrows (e.g., Madarosis)
  • Densification for naturally thin, sparse brows

When Not Suitable

  • In cases of active inflammation (FFA in active phase) — inflammation must be stopped first
  • In active trichotillomania — risk of renewed pulling
  • With unrealistic expectations — the brow line follows natural anatomy

Comparison with Microblading and PMU

There are several options to replace missing eyebrows. An honest comparison:

Eyebrow Transplant

  • Real, natural hairs
  • Permanent result
  • Grow, require trimming and shaping care
  • Can be colored
  • Highest effort, but a one-time procedure
  • Approximately 8–12 months for the final result

Microblading

  • Tattooing of fine strokes that mimic hairs
  • Lasts 1–3 years, then fades
  • Immediate result
  • Low individual effort, but regular touch-ups necessary
  • Appears recognizable as a tattoo up close
  • Can be combined with transplantation

Permanent Make-up (PMU) / Ombré Brows

  • Area-based pigment tattooing in the brow shape
  • Lasts 2–5 years
  • Immediate result
  • Pigments can change color (often gray-blue-red)
  • Corrections are more difficult

Which Choice When

  • Permanent solution desired → Transplantation
  • Quick, temporary result → Microblading
  • Very severe thinning with hardly any residual hair → Transplantation
  • Testing the desired look before transplantation → Microblading as a "trial run"
  • Maximum result → Transplantation plus Microblading (shading between hairs)

Technique and Precision

Eyebrow transplantation is the most technically demanding form of hair transplantation. Each follicle must be placed at the exact angle and correct growth direction — even small deviations are immediately visible.

Our Method

We work with the FUE method using the finest sapphire blades. The advantages for eyebrows:

  • Very fine micro-channels at a precise angle — crucial for natural growth direction
  • Minimal tissue trauma thanks to sapphire blade
  • High implantation density possible
  • Only a local mini-shave of the donor area is necessary

Anatomical Growth Directions

An eyebrow is not a uniform field of hair — the growth direction changes precisely:

  • Inner brow head — hairs grow upwards and slightly outwards
  • Middle brow body — hairs grow diagonally upwards and outwards
  • Brow arch (highest point) — hairs grow sideways outwards
  • Outer brow tip — hairs grow outwards and downwards

These angle variations must be precisely reconstructed. A poorly placed eyebrow immediately reveals itself.

Which Follicles Are Used

We exclusively use FU1 units (single hairs). FU2 or FU3 would look clumpy. The donor area at the back of the head easily provides these single hairs; sometimes we also use fine hairs from the nape of the neck.

Procedure Duration

2–4 hours, outpatient, under local anesthesia. Very fine, precise work — not a quick operation.

Brow Design

The design determines whether the brows look natural or artificial. We follow classic anatomical reference points.

The Three Points of Brow Shape

  • Brow head — begins vertically above the inner corner of the eye (perpendicular from the nostril)
  • Brow arch (highest point) — usually 2/3 from the brow head, vertically above the outer edge of the iris when looking straight ahead
  • Brow end — on an imaginary line from the nostril across the outer corner of the eye

Shape and Facial Proportion

  • Round face — more angular brows with a pronounced arch for structuring
  • Square face — softer, curved brows for softening features
  • Long face — flatter, less arched brows for horizontal emphasis
  • Heart-shaped face — gently arched brows for balance

Symmetry vs. Asymmetry

As with the hairline: conscious slight asymmetry looks more natural than perfect symmetry. We orient ourselves to your natural facial asymmetry.

Pre-Planning with Marking

On the morning of the surgery, the brow shape will be marked on your skin with a white pencil. You give your consent before starting — if necessary, we can still adjust the shape.

Number of Grafts

Typically 100–400 grafts (all FU1) are implanted per eyebrow. The number depends on the initial findings:

  • Slight densification of an existing brow: 80–150 grafts/side
  • Medium restoration: 150–250 grafts/side
  • Complete reshaping: 250–400 grafts/side
  • Very long, dense brows (e.g., Asian/Mediterranean look): 350–450 grafts/side

Total for both brows typically 200–800 grafts. Compared to scalp transplantation: small procedures, but highest precision requirements.

What Influences Graft Count

  • Existing brow hair density
  • Desired density and shape
  • Hair color vs. skin color — less density needed with high contrast
  • Facial anatomy

Aftercare and Results

Day 1–7

  • Small crusts at implantation sites — dissolve after 5–7 days
  • Slight swelling possible, subsides after 2–3 days
  • Careful cleaning with lukewarm water from day 2–3
  • No make-up application in the brow area for 7 days
  • Sleep with the upper body elevated

Week 2–8

  • Shedding phase — the transplanted hairs fall out
  • Brows temporarily look sparse again — normal
  • Make-up or microblading can be used temporarily

Month 3–4

  • First new growth visible
  • Brows slowly become denser again

Month 6–8

  • Significant progress
  • Brows noticeably in final shape

Month 8–12

  • Final result achieved
  • Complete density and natural appearance

Long-Term Care

The transplanted eyebrow hairs grow like head hairs — they become longer than natural eyebrow hair. Trimming every 2–3 weeks is mandatory for a well-groomed look. They can be colored, shaped, and supplemented with an eyebrow pencil.

Duration of Results

Permanent. The transplanted follicles retain their characteristics for life (donor dominance).

Frequently Asked Questions

Do transplanted eyebrows grow like real ones?

They grow faster and longer than natural eyebrow hair — because the follicles originate from scalp hair and retain its growth characteristics. Regular trimming every 2–3 weeks is therefore mandatory.

What is better — eyebrow transplant or microblading?

Depends on the goal. Transplant: real hair, permanent, highest investment, 8–12 months for final result. Microblading: tattoo, lasts 1–3 years, immediately visible, regular touch-ups necessary. For very severe thinning, transplantation is superior; for minor corrections, microblading is often sufficient.

How many grafts do I need per brow?

Typically 100–400 per side, depending on the initial findings and desired density. Precise planning after individual consultation.

Do the brows look good immediately after surgery?

No. Immediately after surgery: crusts, slight redness. After shedding (week 2–8): patchy again. Visible new growth from month 3–4. Final result month 8–12. Patience is required.

Can I still get a transplant after microblading?

Yes. The microblading pigment does not affect the transplant. However: During healing, you should avoid new microblading sessions — at the earliest 6 months after transplantation.

Can I dye my eyebrows after surgery?

Yes, from week 4 after surgery. Take a break before then — chemical dyes can impair healing.
