Washing hair after transplantation - instructions and avoiding mistakes

Washing incorrectly is one of the most common mistakes after a hair transplant – and one of the few you can truly control yourself. When, how often, and with what technique you should wash to ensure the grafts take successfully.

Why Proper Washing Is Crucial

During the first two weeks after transplantation, the scalp is particularly sensitive. The newly implanted grafts are not yet fully anchored, micro-channels are healing, scabs are forming and need to be softened in a controlled manner. Incorrect washing can mechanically dislodge grafts, delay healing, or measurably reduce the growth rate.

The good news: With a simple routine, you can manage this without problems.

The First Wash

Day 0–2: Avoid Water Contact

For the first 48–72 hours, the scalp should remain dry. No showering in the head area, no contact of the implants with water. If necessary: Shower your body, keep your head dry.

Day 3 (or as per clinic instructions): The First Wash

The first head wash usually takes place on the third day, often at the clinic or following precise instructions at home. It has two goals: initial cleaning of blood residue and the beginning of controlled scab dissolution.

Step-by-Step Guide

  1. Preparation: Apply nourishing lotion or prescribed nourishing oil — leave on for 15–30 minutes to soften the scabs
  2. Moistening: Gently let lukewarm water (hand-warm) run over the scalp. Do not use a shower head directly from a short distance
  3. Lather: Lather a mild shampoo (pH-neutral, sulfate-free, fragrance-free) in your hand — never directly on the scalp
  4. Application: Gently dab the foam onto the scalp with your fingertips. DO NOT rub, DO NOT massage, DO NOT scratch with your nails
  5. Soaking time: Leave on for 1–2 minutes — the foam does the work, not the movement
  6. Rinsing: Rinse again with lukewarm, gently flowing water. Do not shower your head from 20 cm height
  7. Drying: Only pat dry with a soft towel — never rub. Air dry or use a cool hairdryer setting from 30 cm distance

Which Shampoo?

  • Mild, pH-neutral shampoo — many clinics recommend baby shampoo (pH 5.5)
  • Sulfate-free (no SLS/SLES) — gentler for healing
  • Fragrance and preservative-free — reduces irritation
  • No medicinal shampoos (for dandruff, antifungals) for the first 4 weeks without medical approval
  • No caffeine or "anti-hair loss" shampoos — active ingredients can irritate healing

How Often to Wash?

  • Days 3–14: once daily, always with the same gentle routine. This dissolves scabs in a controlled manner and reduces bacterial load
  • Days 14–30: once daily, gradually make the foam denser and leave it on for a shorter time
  • From day 30: normal routine, gentle massage allowed
  • From week 6: normal shampoos, hair care products, styling aids allowed

Common Mistakes

  • Washing too early (before day 3) — can dislodge grafts
  • Too strong a water jet directly on the recipient area
  • Rubbing instead of dabbing — mechanical stress for the grafts
  • Forcibly removing scabs with a comb or brush — can also dislodge follicles
  • Hot water — promotes swelling, weakens fresh healing
  • Aggressive shampoo with sulfates and fragrances — irritates healing
  • Rubbing with a towel — a classic that can cost grafts
  • Swimming or sauna before week 6 — chlorine, heat, and sweat impair healing

Special Cases

If scabs do not come off

Some scabs persist longer than the typical 10–14 days. If you attempt mechanical removal too early, the freshly anchored graft can come off with it. Solution: continue to soak with nourishing oil, let water work gently, the body will dissolve the scabs on its own by day 18–21. In exceptional cases, the clinic may perform gentle removal.

For itching

Itching is a normal sign of healing — especially from week 2 onwards. Please do not scratch! Allowed: light tapping with fingertips, cool compress, possibly a mild antihistamine after consulting your clinic.

For beard transplant or other locations

The same principles apply — adapted to the region. For beard: shaving only from week 4, carefully electric (not wet with a blade). For eyebrows: only from day 5 gently dab with a damp cotton pad.

Frequently Asked Questions

What if I lose a graft while washing?

Unlikely with correct technique. If it happens — sometimes a single graft comes off with the scab after day 5–7. If the loss affects a single unit, it is not problematic. In case of greater loss (multiple grafts) or bleeding: contact the clinic immediately.

Can I use dry shampoo?

No, not in the first 4 weeks. Propellants and aluminum salts can irritate healing. Possible with restrained use from week 4.

Can I rinse with conditioner?

It is better not to in the first 2 weeks — silicones and conditioning agents can settle in the healing micro-channels. Light application to the lengths is allowed from week 3, not directly on the implants.

What if I forgot to wash once?

No drama. Skipping once is not a problem. Regular gentle care is important, not the perfection of individual days.

Do I have to apply the nourishing oil before every wash?

Yes, in the first 10–14 days, then according to clinic instructions. The oil softens the scabs and protects the healing areas.
